1.  Nurse-Led Care:
Paediatric nurses and highly trained support staff

Rapid In-Home Crisis Support
Highly trained children’s nurses and specialist healthcare professionals deliver immediate, compassionately responsive care within the home or placement, providing:

Clinical monitoring and symptom management
Medication administration and escalation planning
Personal care and daily living support
Crisis stabilisation to avoid unnecessary hospital admission

2. Specialist Clinical Interventions
We support children with highly complex medical needs, including:

Tracheostomy, Cough Assist, {NIV} non-invasive ventilator-dependent care
Enteral feeding, TPN, and PEG management
Complex neurological conditions and disabilities
Rare and genetic disorders
Palliative and end-of-life care
All care is delivered in line with evidence-based protocols, individual risk assessments, and safeguarding frameworks.

3. Family involvement for a holistic approach at home
Individually Tailored Care Planning Each child receives a bespoke, outcomes-focused care plan, co-produced with:

 

The child and family
Referring clinicians and crisis teams
Education and therapy services where required
Plans are dynamic, regularly reviewed, and responsive to changing clinical and psychosocial needs.

Age 0-17 years

Crisis-Responsive, Home-Based Clinical Support

Select-12 Children’s Complex Care delivers high-acuity, nurse-led, personalised care for children and young people with complex, life-limiting, or life-changing health conditions, providing a safe and effective alternative to hospital admission or prolonged inpatient stays.

 

Our Children Complex Care service is designed to stabilise crises, prevent escalation, and support timely discharge by delivering clinically robust care in the child’s home or placement, while preserving family life, education, and emotional wellbeing.

We work in close partnership with Children’s Crisis MDTs, ICBs, NHS providers, Local Authorities, and safeguarding professionals to ensure rapid mobilisation, continuity of care, and risk-managed outcomes.
